The Seattle Municipal Archives has posted the cover. They've also posted a copy of the map, but it's not quite legible at their resolution.
This digital copy is slightly deformed because of the automatic stitching process. It was scanned in pieces. The blurriness is an artifact of the original printing, though. If you can't tell what spot they're talking about, look it up in the index.
